As an Accountable Care Network, People of Color Network, Inc. (PCN), a leading community-based 501c(3) charitable organization, is providing integrated behavioral and physical health services through collaborative care programs, culturally and linguistically responsive services, serving more than 6,000 adults, youth, children and their families in Maricopa County and parts of Pinal County.
PCN is comprised of culturally competent, quality focused, community‐based behavioral and physical health providers to deliver integrated community heallthcare to achieve its mission of "Creating and Maintaining Healthy People of Color Communities by Turning Recovery Into Living Well."
Awarded in 2012 as the Top Provider Network and Clinic, PCN serves adults, youth, children and families challenged with serious behavioral health and mental illness, addictions and chronic health conditions to help close the gap in service disparity to better meet the needs of diverse communities by its founding members Centro de Amistad, Chicanos Por La Causa, Ebony House, Inc., Friendly House, Inc., EMPACT-Suicide Prevention Center (SPC), and Native American Connections (NAC).

Our brand transcends the communities of color we serve throughout Maricopa County guiding our focus of meeting the whole health needs of people and communities. PCN is mindful a brand identity is more than a graphic design and colors; it reflects an overall experience people have with our organization – creating relationships built on trust, integrity and the proven ability to provide exceptional service.
Our new logo design elements convey attributes of our organization’s vision, mission and core principles as well as the culturally and linguistically responsive services we offer to the children,
youth, families and adults we are privileged to serve. Collaboration, advocacy and cultural competency are
hallmarks of People of Color Network connecting the organization's founding
